RULES FOR THE GIVEAWAY AND OTHER INFORMATION

1. This monthly promotion:
   • Is open to all people living in Minnesota who can include family members living in neighboring states (Iowa, Wisconsin, North Dakota or South Dakota).
   • Is for a limited time while supplies last. 
   • Is designed for new participant families ONLY. 
   • Does not replace the benefits participants receive for completing the individual steps to join the study. 
2. Maximum of one gift per participating family, no matter the number of family members that enroll in the study.  
3. Participation means that:
   • A minimum of two family members must enroll in the study.
   • The first family member must be at least 18 years old and live in Minnesota. Other family members may live in Minnesota or any neighboring states (Iowa, Wisconsin, North Dakota or South Dakota).
4. Enrollment steps:
   • First participant:
     • Fill out the eligibility survey
     • Invite at least one family member and/or register children under 18 years old.
   • Each family member needs to complete a health questionnaire (Parents fill out the health questionnaire for children under 18 years old).
   • Each participant needs to sign an eHIPAA and a participation eConsent to join the study. 
   • Complete a family health visit and return the environmental exposure tests.
5. To claim the gift:
   • After at least two family members sign the eHIPAA and eConsent, and attended a Health Visit, the family member claiming the gift needs to email [email protected]. 
     • Please send us your mailing address, the family ID we assigned to your family (or the full name of the first participant), and include the name of the gift you are claiming.
     • We will email you when your gift is in the mail. Shipping is done twice per month. Please allow a few weeks after you complete the process to receive it. We appreciate your patience 
     • Please remember that if the gift is out of stock, we might replace it with one of a similar price. No money will be given in exchange.
